Correlates of Diabetes in Elderly Population

Background and Aim: Diabetes Mellitus (DM) incidence is growing worldwide and is turning into an epidemic and endemic trouble including the social or financial burden with higher mortality in aged people. This study was aimed to detect the predictors of DM and pre-diabetes in geriatric group of population of Shiraz, Iran. Materials and Methods: 412 elderly people who were under coverage of public health centers were selected by a multistage cluster randomized sampling and a valid and reliable questionnaire was filled for each of them through in-person interview. Acquired data consisted of demographic, socioeconomic, clinical and laboratory data. Data was analyzed in SPSS using parametric, non-parametric tests and logistic regression model considering p value <0.05 as significant level. Results: Mean age of participants was 68.1 ± 6.2 and male (196) to female (216) ratio was 0.9. Out of 412, 288(69.9%) were married and 282 (68.4%) were illiterate or educated up to 5 years. 137 (33.2%) were diagnosed as diabetic cases including 128 (31%) known cases of DM and 9 (3.3% ) new detected cases of DM while 275 (66.7%) were diagnosed as pre-diabetics. Univariate analysis showed that low education (p=0.03), supplement insurance (p=0.04), hypertension (p=<0.001), hyperlipidemia (p=<0.001), hepatobiliary disease (p=0.003), sexual dysfunction (p=0.05) and quality of life (p=0/01) were higher in diabetic versus prediabetics. Multivariate analysis showed that hyperlipidemia (OR=3.5), hepatobiliary (OR=3.1), HTN (OR=1.9) and low education (OR=5.0) were in order associated with DM. Conclusion: Pre-diabetes is common among elderly population and this finding address to implement appropriate intervention regarding screening of pre-diabetes and its proper management in this group of population. Moreover, diabetes with hyperlipidemia, hepatobiliary and HTN especially in low educated cases should be more under care of public health centers.
