Background and Objective: Melasma is a common acquired pigmentation of sun exposed area, occurring most commonly on the face, which is slowly and symmetrically Develops. It is common in reproductive ages and associated with pregnancy, and is therefore referred to as a gestational mask. Progesterone and estrogen stimulate melanogenesis, which is why they are known to increase pigmentation during pregnancy and also use of contraceptive pills. The aim of this study was Comparison between the serum levels of 5-Alpha-Androstenedione and insulin hormones in women with melasma and without melasma. Materials and Methods: The present study was a case-control study. The study population was women over 27 years old without any specific metabolic disease that they were admitted in dermatologic clinic. 141 women with melasma as case group and 141 women without melasma as control group were entered in study. Melasma Area and Severity Index were used to evaluate the severity of melasma. After blood sampling, the serum was isolated by centrifugation and stored in a refrigerator at a temperature of minus 20 °C. Serum levels of 5-Alpha-Androstenedione and insulin were measured by using insulin and 5-Alpha-Androstenedione kits IBL manufacturing by ELISA method and serum glucose levels were measured with the Pars zinc glucose kit by biochemical device. T-test independent, bivariate correlate and Chi-square test were used for data analysis. P value less than 0.05 was considered statistically significant.Results: The mean age of participants in the case group was 31.9 ± 3.2 and in the control group 31.1 ± 3.8 years. The mean serum insulin level in case group was 7.07 ± 2 and in control group was 7.05 ± 2.2, and this difference was not statistically significant (P = 0.95(. The mean serum level of 5-Alpha-Androstenedione in the case groups was 3 ±1.3 and in the control group was 1.7 ± 0.9. This difference was statistically significant (P <0.001). The mean serum level of FBS in the case group was 92.47 ± 8.5 and in the control group was 93.46 ± 8.02. This difference was not statistically significant (P = 0.001). Conclusion: Our findings showed that the mean serum level of 5-Androstenedione) is a prohormone of testosterone (higher in the case group. Higher levels of this hormone may suggest a potential association in the etiology of melasma.
