Clinicopathological study of liver tumors comparison with sonographic findings (A cross-sectional, analytical study)

Background: Early and correct diagnosis of liver tumors by non-invasive methods such as ultrasound could prevent unnecessary surgeries. Aims: To determine the relationship between clinical symptoms, ultrasound and pathology findings in the diagnosis of space-occupying lesions of the liver. Methods: This cross-sectional analytical study was conducted on all liver biopsy samples sent (during 2003-2013) to the pathology department from November 2013 to July 2014. Age, gender, clinical symptoms, initial clinical diagnosis, pathology reports and ultrasound results were considered as variables. Data were analyzed by using SPSS version 17. Results: Of 164 patients with liver biopsies 87(53%) were men. The mean age for malignant and benign liver lesions were 57.9±17.2 and 44.9±19.4 respectively. The most common primary origin of metastasis was gastrointestinal tract tumors. In differentiating malignant and benign lesions, the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were measured as, 38.1% 88.2%, 96.6%, and 14.1% for the initial clinical diagnosis, and 89.4%, 8.3%, 79.2%, and 16.7% for the ultrasound. Conclusion: When a lesion is found malignant in the initial clinical diagnosis and the ultrasound, we can accept it with more confidence, but negative results of ultrasound; need more clinical evaluation and investigations.
