Frequency Survey of Endometrial Hyperplasia and Its Risk Factors Among the Premenopausal Women With Abnormal Uterine Bleeding in Tehran, Capital of Iran

Introduction: Abnormal vaginal bleeding (AUB) is one of the most common presenting gynecologic complaints in women seeking gynecologic health care regardless of age. Although the differential diagnosis includes a wide range of pathologies such as endometrial and endocervical polyps, myomas, synechiae, uterine malformations, endometrial hyperplasia (EH) and endometrial cancer, the vast majority had benign causes.This complaint is taken more seriously when it occurs in women of late reproductive age due to higher possibility of malignancies. Studies showed EH is more frequent pathologic findings in the histopathologic specimens of premenopausal women. Considering the wide variety of data on EH among Iranian women, this study was aimed to frequency survey of EH and its risk factors among the premenopausal women with AUB. Material and Methods: This cross-sectional study was performed on the premenopausal women aged 45-50 ears with the chief completion of AUB who wereattended to the Bou-Ali Hospital, Tehran, during 2005-2015. Demographic and medical history of patients such as the familial history of EH, nulliparity, any metabolic disorders, received Tamoxifen and family history of breast cancer were recorded. A gynecologist and examined all the patients and dilation and curettage (D&C) was performed under general anesthesia. The pathologic finding were reported, and the frequency of EH in the pathological sample was evaluated. Results: The mean age of the patient was 47.41±1.71 years. The mean duration of AUB was 5.9±1.86 months. Ten patients (20.2%) was nulliparous. Diabetes Mellitus (DM), hypertension, and hypothyroidism were observed in 52(11.5%), 95(21%) and 68(15%) of patients, respectively. Also. The positive family history of EH and breast cancer were seen in 25(5.5%) and 8 (1.8) of patients, respectively. Four patients(0.9%) had a history of Tamoxifen intake. Discussion: Regarding our results, its seem that the frequency of EH among premenopausal women was higher than age-group and clinician should be awarded and considered any women with AUB for more evaluation and screening of possible cancer. Also, the nulliparous, DM and positive family history were the most important risk factor of EH.
