Cyclin D3 overexpression in Hurthle cell carcinoma

چکیده مقاله:

Introduction: Hurthle cell thyroid carcinoma cannot be diagnosed reliably by Fine needle aspiration(FNA) technique. so in present study cyclin D3 overexpression usingimmunohistochemistry on histologic samples of Hurthle cell carcinoma to assess the diagnostic value of cyclin D3 immunohistochemistry beside cytology. Methods:twenty-six FNA samples of suspicious for hurtle cell carcinoma and that 8 malignant cases of hurtle cell carcinoma were examined. Cyclin D3 expression levels were evaluated in acquired samples and were compared with levels of the closely related cyclin D1 protein. Specificity, sensitivity, negative predictive value (NPV), positive predictive value (PPV), and diagnostic accuracy were calculated using standard formulas for each sample separately. Results: Based on previous studies higher than 25 % was considered as cutooff point. Specificity for cyclin D1 and cyclin D3 were highlabout 100% for both and accuracy was 82% and 95% respectively in distinguishing between malignant and benignhurthle neoplasia; Both cyclins D3 and D1 had low sensitivity which were 78% and 34% respectively and low NPV which were 68% and 84%, respectively. PPV for each was 100%. Cyclin D3 (higher than 7%) and cyclin D1 (higher than 6%) were found to be highly sensitive (100% for both) and accurate ,which were 90% and 94%, respectively; In other hand, cyclins D1 and D3 had low specificity 80% and 89%, respectively; and a low PPV which were 79% and 86%, respectively.Conclusion According to the results ,high cyclin D3 level is in favor of malignancy in indeterminate hurthle neoplasias.
