Inguinal Actinomyces Lymphadenitis: Case report

چکیده مقاله:

Introduction:Actinomyces is a gram positive anaerobic filamentous bacilli usually found in pelvic inflammation in association with intrauterine device. The lymph node involvement is rare and among 47 reviewed cases of Actinomycosis infection only 3 cases of actinomyces lymphadenitis of the cervical or mediastinal have been reported sofar.To the best of our knowledge this is the first case of inguinal Actinomyces lymphadenitis. Case presentation: We report a 45 years old afghan man presented with pain, swelling and purulent discharge of right knee with a history of multiple ulcerated skin lesions since 2.5 years ago, which initially diagnosed as septic arthritis but had no response to antibiotic therapy. Ultrasonography showed right inguinal hypoheretroechogenic mass which composed of lymphoid tissue involved by a filamentous branchingbacteria containing sulfur granules surrounded by numerous histiocytosis and neutrophils on light microscopy. The patient underwent medical therapy and discharged in good general condition. Conclusion: Actinomyces should be included in the differential diagnosis of persistent lymphadenopathy .The accurate diagnosis requires surgical excision for pathologic evaluation and microbial testing.
