Background:Urinary tract infection (UTI) is a main cause of neonatal fever. But the diagnosis of UTI canbe missed because the symptoms are often non-specific and the sterile sampling may bedifficult. In the neonate the clinical approach and radiologic investigations of UTI are quitedifferent from that of older children and adults. Early diagnosis of neonatal UTI and detectingof the underlying congenital genitourinary tract anomalies can prevent recurrent kidneydamage and end stage renal disease. Our objective is to determine the common bacteriaspecies isolated from urine culture, and their drug resistance in the Kosar Hospital.Patients and Methods:For this cross-sectional study data were evaluated from 21 babies with positive urine culturereports that admitted at the Kosar Hospital between March 2010 and December 2012. Thedata from these participants were obtained by a check list and review of laboratory reportsheets of urine culture results and antibiograms. Both descriptive and statistical analysismethods were applied.Results:Eleven subjects were boys. Six of the 21 subjects had low birth weights (< 2,500 gm). Theirmean gestational age was 34 ± 3/7 weeks. The most common complaint was fever (83%),followed by poor appetite, diarrhea, vomiting and abdominal distention. Pyuria were foundin 17 patients. Escherichia coli was the most common pathogens, found in 14 cases, followedby Klebsiella pneumonia (2 cases), Enterococcus (3 cases) and Enterobacter cloacae (2cases).Conclusions:The clinical presentations of UTI in neonate patients are nonspecific and urinary tractobstruction manifestations often followed by repeated UTI. Physicians need to be alert andsensitize to these important clinical presentations. Also any UTI in neonate patients should beworked up for urinary obstruction or reflux. Further prophylactic approach, either medical orsurgical, is performed to decrease the renal damage.
