Composite diffuse splenic capillary hemangioma and splenic cyst

Cystic lesions of spleen are devided into parasitic and non-parasitic cysts. Parasitic cysts aremostly due to echinoccocal disease and account for 60-70% of splenic cysts in countrieswhere hydatid disease is endemic. Non-parasitic splenic cysts are classified as primary (true)cysts, which have an epithelial lining and secondary cysts (pseudocyst) without lining and areusually of a post-traumatic etiology. Splenic hemangioma is the most common benign tumorof spleen. Composite diffuse capillary hemangioma and cyst of spleen are extremely rare.Hereby, we report the coexistence of these two conditions in two cases. Case 1 is a 14-yearoldgirl who was admitted to the hospital with history of splenic cyst since two years beforeand had undergone laparascopic aspiration twice, which was bloody fluid at cytologicalexamination. Abdominal ultrasonography and computerized tomography scan showed a13x11 cm splenic cystic mass in the lower part of spleen. At microscopic examination, thecyst was partly lined by a single to more layers of flat to cuboidal cells to squamous cells.The cyst was diagnosed as epithelial cyst.Case 2 is a 2.5 year-old boy with a unilocular 8 cm cyst in the spleen which was detected atabdominal sonography, performed for evaluation of inguinal hernia. Microscopicexamination showed a fibrous cyst wall lined by a single layer of flat cells. The residualsplenic tissue, in both cases, was diffusely involved by small size blood vessels.To the best of our knowledge, composite diffuse splenic capillary hemangioma and spleniccyst is not previously reported.
