Granulomatous interstitial nephritis as a manifestation of Crohn’s disease: A case report

Granulomatous interstitial nephritis is a rare extraintestinal manifestation of Crohn’s diseasethat has been described in four patients previously. We report a 23-year-old male with historyof Crohn’s disease since age six who was admitted to the hospital for weight loss, fever andbloody diarrhea in the midst of a recent flare over the past two months. Investigationsrevealed anemia, high ESR, high CRP and an elevated serum creatinine level.Histopathological examination of tissue specimens obtained at renal biopsy demonstratedgranulomatous interstitial nephritis. Crohn’s disease needs to be in the differential diagnosisof granulomatous interstitial nephritis and can be a manifestation of drug allergy or theCrohn’s disease itself.
