Introduction and Aim of the Work. Patients in our society have 2 important criteria:1) lack of early diagnosis, 2) topmost consideration for pregnancy and motherhood. Therefore, unfortunately, not few breast cancer patients present in late pregnancy and in terminal and dying states. Special medical and psycho-moral support is strongly needed for these patients and their partners. The work aims at defining and evaluating the effectiveness of such system. Material and Methods. Thirty one patients of terminal and dying 4th stage breast carcinoma were managed in their 3rd trimester pregnancies. Management was through special composite care program (SCCP) that entailed: 1) palliative medical interventions, with analgesia and palliative chemotherapy, 2) psycho/moral support program, 3) pregnancy care, 4) proper delivery, 5) postpartum and neonatal care program, and, 6) family and special support group care. Results. All patients insisted on preserving their pregnancy as most as possible. Allgave birth after the 36th week. All women died before the 6th month postpartum; one died 3 days after labor. Thirty live fetuses and one case of fetal demise resulted. Painand psycho scales were positively reduced by the SCCP. The most valuable elements were: 1) the moral support, 2) offering the best neonatal care. Conclusion. Terminal breast patients benefited significantly from the SCCP. Fetal and neonatal care proved most important. Application of parallel programs to terminal pregnant women with other cancers is to be considered.
