Studying and Identifying of Diving Operations Risks and its Remedies (Case Study: Investigation Of Drowning Casualty of Researching Vessel Master During Diving Operation)

Despite repeated efforts on building safety vessels and using advanced equipment such as navigation equipment and safety equipment, marine accidents still occur, resulting in personal injury and property damages and pollution of the marine environment. Analyses of casualties in marine and aviation transportation industries indicate that human error is a major contributor in accidents and incidents. Recent researches in this field broadly identify various types elements that contribute to human error. Accordingly particular managerial measures and steps have been taken to mitigate or control the severity of those errors. In this paper according to the casualty resulted death of researching ship master, based on a qualitative research and utilizing related casualty investigation models and techniques, latent and contributory factors, which associated with human error, will be identified and required corrective action will be suggested, hence all can lead to prevent or reduce similar disasters and to increase the safety of diving operations
