چکیده مقاله:

Introduction: Trauma-related injuries are a leading cause of hospitalization and mortalityworldwide. In response to trauma, pathological and anti-inflammatory processes occur, characterized by an increase in neutrophils and a decrease in lymphocytes. This study aimed toassess the prognostic value of the neutrophil-to-lymphocyte ratio (NLR) in predicting outcomesin multiple trauma patients, focusing on hospital stay length and mortality. Methods: This study enrolled ۴۷۴ adult trauma patients (aged ۱۴ to ۸۰) who had referred tothe Qazvin Provincial Trauma Center in the first half of ۲۰۲۴ using the convenience samplingmethod. The Injury Severity Score (ISS) was used to classify patients, and data includingcomplete blood count (CBC) and differential count (Diff) tests were collected. Statistical analysiswas conducted using the Kolmogorov–Smirnov test for data normality, the chi-square test forqualitative variables, and Pearson’s correlation coefficient for quantitative variables. Dataanalysis was done using SPSS version ۱۶.Results: Of the patients, ۶۷.۷% were male, and the mortality rate was ۹.۷۲%. The average NLRwas ۳.۹, with a cutoff point identified by maximizing the Youden index. Cox regression analysisshowed that NLR > ۳.۹ was significantly associated with prolonged hospital stays P < ۰.۰۰۱) andICU admissions (P = ۰.۰۰۴). Additionally, ۶۹% of the patients who died had an NLR greater than۳.۹ (P < ۰.۰۰۱).Conclusion: In conclusion, NLR > ۳.۹ serves as a reliable prognostic marker for adverseoutcomes, including prolonged hospitalization and increased mortality. NLR is a simple, costeffectivetool that could be implemented in clinical settings to predict patient prognosis andguide more effective interventions. Further research is needed to confirm these findings andinvestigate the mechanisms by which NLR influences trauma outcomes.
