چکیده مقاله:
Objectives: Stroke is the most common cause of long-term disability, and people with stroke often have trouble with balance and motor skills. This study aimed to determine whether an ۸-week, multi-component neuromuscular balance training program or regular physiotherapy was better at helping people with chronic stroke improve their motor coordination and body control.
Methods: This assessor-blinded randomized controlled trial included ۵۲ people who were placed into either a progressive muscle program (n=۲۶) or time-matched standard physiotherapy (n=۲۶). The Fugl-meyer assessment of lower extremity (FMLE) score was the main clinical outcome. The Berg balance scale (BBS), timed up and go (TUG) test, and ۱۰-meter walk test (۱۰MWT) were the secondary outcomes.
Results: A significant group×time association was observed for all results. Motor coordination (FMLE change: ۵.۱۴±۱.۶۳ points vs ۱.۷۶±۰.۶۹ points; P<۰.۰۰۱; Cohen’s d=۲.۶۹), balance (BBS change: ۷.۹۱±۳.۱۱ points vs ۲.۲۵±۰.۷۸ points; P<۰.۰۰۱; d=۲.۴۹), functional mobility (TUG change: −۳.۲۱±۱.۳۹ s vs −۱.۱۴±۰.۵۸ s; P<۰.۰۰۱; d=۱.۹۴), and gait speed (۱۰MWT change: ۰.۲۱±۰.۰۶ m/s vs ۰.۰۸±۰.۰۴ m/s; P<۰.۰۰۱; d=۲.۵۵) all improved in the intervention group. In the intervention group, better motor coordination was strongly linked to better balance (r=۰.۶۸, P<۰.۰۰۱) and functional movement (TUG change: r=-۰.۵۹, P=۰.۰۰۱).
Discussion: In conclusion, a focused neuromuscular program significantly improves muscle coordination and balance after a stroke. These results show that programs that specifically teach sensorimotor integration are very important for helping people who have had a stroke improve their daily lives.
