چکیده مقاله:

Background & Objective: Pregestational diabetes is a leading preventable cause of congenital anomalies. We characterized Streptozotocin (STZ)-induced insulin-deficient diabetic embryopathy and compared the effects of insulin glargine, metformin, and their combination.Materials & Methods: Forty-eight time-mated Sprague-Dawley dams were allocated by blinded block randomization into six groups (n=۸ litters per group): normal control, citrate-buffer vehicle control, STZ-induced diabetic, and STZ-diabetic treated with insulin glargine (۲ IU/kg/day s.c.), metformin (۲۰۰ mg/kg/day p.o.), or their combination from Gestational Day (GD) ۸ to ۲۰. Diabetes was induced via STZ (۵۰ mg/kg i.p.) on GD ۷. Fetuses were harvested on GD ۲۰-۲۱ for morphometric, histopathological, biochemical, and molecular analyses, with a satellite cohort providing GD ۱۲-۱۸ data. Results: Diabetic dams developed hyperglycemia (۳۸۹.۲±۲۲.۶ mg/dL at GD ۲۰), a fetal malformation prevalence of ۴۴.۳% (۹۵% CI ۳۸.۱-۵۰.۷), and a fourfold increase in placental malondialdehyde levels. Pax۳ and Sox۲ transcripts decreased to ۳۱% and ۲۸% of control values, respectively, while cleaved Caspase-۳ levels increased ۴.۸۲-fold. Combined therapy provided the greatest protection, restoring fetal weight to ۹۱.۲% of control and reducing malformation prevalence to ۴.۷% (۹۵% CI ۲.۴-۸.۶), significantly outperforming insulin monotherapy in reducing malformations and modulating Pax۳, Nrf۲, and Caspase-۳ levels. Conclusion: This model produced reproducible embryopathy associated with placental oxidative stress, neuroepithelial apoptosis, and transcriptional dysregulation of morphogenesis. Combined therapy provided superior restoration compared with either monotherapy, supporting complementary glycemic and non-glycemic mechanisms; however, further confirmation is warranted before clinical extrapolation.
