Background & Objective: Polycystic Ovary Syndrome (PCOS) is a common, chronic endocrine disorder of reproductive-aged women that can reduce quality of life. Transforming Growth Factor-Beta ۱ (TGF-β۱) is involved in ovarian fibrosis and folliculogenesis, and TGFβ۱ gene polymorphisms may influence susceptibility. This study was conducted with aim to determine the frequency of the TGFβ۱ C/T genotypes in women with PCOS and to assess whether the polymorphism is associated with serum TGF-β۱ levels and PCOS. Materials & Methods: This cross-sectional study was conducted on ۱۰۰ women with PCOS diagnosed by the Rotterdam criteria and ۵۰ healthy controls aged ۲۰-۴۵ years. Serum TGF-β۱ was measured by ELISA, the TGFβ۱ polymorphism was genotyped by PCR and Sanger sequencing, and hematological indices and ABO/Rh blood groups were recorded. Data were analyzed using SPSS (version ۲۶) and independent-samples t-test and chi-square test. P<۰.۰۵ was considered statistically signifcant. Results: Serum TGF-β۱ was significantly higher in PCOS than in controls (۷.۶۴±۲.۷۹ vs ۶.۲۷±۲.۱۴ ng/mL; P<۰.۰۱). The CT genotype was the most frequent (۴۳%), but the genotype distribution did not differ significantly (P=۰.۱۱۴), and serum TGF-β۱ did not differ across genotypes. White blood cells, monocytes, lymphocytes and platelets were significantly higher in PCOS, and blood group B-positive was more frequent among patients (P=۰.۰۱۶). Conclusion: Serum TGF-β۱ was elevated in women with PCOS, but the TGFβ۱ C/T polymorphism was not associated with PCOS susceptibility or with circulating TGF-β۱ in this sample. These exploratory findings require confirmation in larger, adjusted studies.
