Rumination, a transdiagnostic cognitive process characterized by repetitive and passive focus on distress, its causes, and consequences, is a well-established risk factor for the onset and maintenance of anxiety disorders. Cognitive Behavioral Therapy (CBT) is a highly effective treatment for anxiety disorders, yet treatment response varies significantly among individuals. This meta-analysis investigates the predictive utility of baseline rumination levels on the longitudinal response to CBT in individuals with anxiety disorders. Drawing upon longitudinal studies published between ۲۰۲۰ and ۲۰۲۵, including both international and Persian literature, this study aims to synthesize existing evidence to determine if pre-treatment rumination can reliably predict post-treatment anxiety symptom reduction and overall CBT outcome. A systematic search of Google Scholar and other relevant databases identified ten eligible studies. Effect sizes, primarily correlation coefficients (r), were extracted and subjected to random-effects meta-analysis. Subgroup analyses explored potential moderators such as specific anxiety disorder diagnoses and CBT modalities (transdiagnostic vs. disorder-specific). The findings indicate a moderate, statistically significant predictive relationship between higher baseline rumination and poorer CBT outcomes, suggesting that individuals with elevated rumination may require tailored interventions to enhance treatment efficacy. This meta-analysis provides crucial insights for clinical practice, emphasizing the importance of assessing rumination at baseline to personalize CBT approaches and potentially integrate rumination-focused strategies for improved patient outcomes.
