Extracts of Lentinula edodes and probiotic-derived postbiotics, as emerging natural biotherapeutics, have attracted considerable attention due to their complementary properties in modulating immune responses and suppressing inflammation. Bioactive compounds of Lentinula edodes, particularly polysaccharides, play a significant role by enhancing the activity of antigen-presenting cells (APCs), regulating cytokine production, and strengthening both innate and adaptive immune responses. Concurrently, postbiotics exert their therapeutic effects through modulation of inflammatory signaling pathways, reinforcement of the epithelial barrier, and regulation of host-gut microbiota interactions. The potential synergy between these two approaches lies in their ability to target multiple levels of immune regulation, thereby attenuating pathological inflammation while preserving host defense mechanisms. In this narrative review, studies published between ۲۰۱۰ and ۲۰۲۶ were systematically searched and analyzed using databases including PubMed, Scopus, and Web of Science, with relevant keywords. This review aims to provide a conceptual framework for the synergistic potential of these two interventions in inflammatory diseases. Despite the limited number of combined (co-intervention) studies, current evidence suggests that integrating these approaches may represent a novel strategy for the development of functional foods and adjunct therapeutic interventions with high immunomodulatory efficacy.
