Children presenting with acute head and neck conditions often require timely imaging to secure a diagnosis and guide urgent management. The choice of modality is influenced by clinical urgency, diagnostic yield, and the need to limit radiation exposure. Conventional radiography has limited use but may be considered for airway compromise, ingested foreign bodies, or suspected cervical spine injury. Ultrasound, given its safety and accessibility, is highly useful for evaluating soft tissue swellings, abscesses, lymphadenopathy, and vascular lesions. Computed tomography is the most rapid and reliable tool for assessing trauma, complex fractures, or deep neck space infections when surgical planning is necessary, although radiation risks require careful justification. Magnetic resonance imaging provides excellent soft tissue detail without ionizing radiation and is especially valuable for intracranial, vascular, or orbital extension of disease, but practical barriers such as longer scan times and the need for sedation limit its emergency role. Overall, an individualized, condition-specific approach-favoring ultrasound and MRI whenever possible and reserving CT for time-critical diagnoses-achieves an optimal balance between accuracy and safety in pediatric emergencies.
