Stridor is a high-pitched, noisy respiratory sound resulting from partial airway obstruction. In children, it is a common clinical sign that may arise from various etiologies, ranging from benign, self-limiting conditions to life-threatening emergencies. Common causes include viral croup, laryngomalacia, epiglottitis, and foreign body aspiration. A thorough history and physical examination are essential for determining the site and severity of obstruction. Diagnostic tools, such as flexible laryngoscopy, radiographic imaging, and pulse oximetry, can aid in assessment. Management depends on the underlying cause and severity, with priorities including airway stabilization, oxygen supplementation, and specific treatments such as corticosteroids, nebulized epinephrine, or surgical intervention. Early recognition and appropriate intervention are critical to prevent morbidity and mortality. This review summarizes the current evidence on the etiology, diagnosis, and management of pediatric stridor, highlighting key points for clinical practice.
