Barriers and Facilitators of Self-Care, Health Information Seeking, and Engagement With Digital Psychoeducational Interventions Among Women With Positive HPV Test Results: A Theory Informed Scoping Review Based on the Digital Cumulative Complexity Model (DiCuCoM)

Receiving a positive HPV result in cervical cancer screening is associated with significant psychosocial consequences including anxiety, distress, stigma, uncertainty, and impaired self care. Digital psychoeducational interventions have the potential to support these women; however, engagement with such interventions faces multiple barriers, and a comprehensive map of these barriers and facilitators based on a specific theoretical framework has not yet been provided. This scoping review aimed to identify, categorize, and analyze the barriers and facilitators of engagement among women with positive HPV results in digital psychoeducational interventions, self care, health information seeking, and care follow up, and to interpret these factors based on the Digital Cumulative Complexity Model (DiCuCoM). This theory informed scoping review was conducted following the JBI methodology and reported according to the PRISMA ScR statement. A systematic search was performed in PubMed, Scopus, Web of Science, and CINAHL for articles published between ۲۰۲۴ and ۲۰۲۶. Quantitative, qualitative, and mixed methods studies that examined HPV positive women or provided separable data for this population and reported at least one digital, psychoeducational, or self care component were included. Data analysis was performed using directed content analysis and mapping of findings into the two dimensions of "demands/workload" and "capacities" within the DiCuCoM framework. From four included studies, barriers were identified in four main categories: individual and psychological burdens, informational and treatment burdens, social and cultural burdens, and technological burdens. Facilitators were categorized into four capacity domains: individual capacity, social capacity, health system capacity, and technological capacity. The DiCuCoM model provides an appropriate framework for analyzing the interaction between "digital and care burdens" and "individual and social capacities" in HPV positive women. The design of digital interventions should focus on reducing identified burdens and strengthening supportive capacities. Significant knowledge gaps exist regarding dedicated digital interventions for this population, necessitating randomized controlled trials and longitudinal studies.
