Complete Molar Pregnancy with a Concurrent Fetus: A Case Report

Molar pregnancy with a coexisting live fetus is a rare and diagnostically challenging condition, particularly when clinical manifestations are subtle. We report a ۳۷-year-old woman who conceived via IVF and presented at ۲۴ weeks of gestation with symptoms of preeclampsia. Ultrasound examination revealed intrauterine fetal demise and a large cystic placental mass initially suspected to represent placental mesenchymal dysplasia. Labor was induced, resulting in the delivery of a nonviable fetus accompanied by grape‑like vesicular tissue. Histopathological evaluation confirmed a complete hydatidiform mole. This case underscores the importance of meticulous ultrasonographic surveillance and timely diagnosis, particularly in pregnancies achieved through assisted reproductive technologies, to optimize outcomes in pregnancies complicated by molar changes.
