Clinical and Microbiological Characteristics of Catheter-Associated Urinary Tract Infections in a Tertiary Hospital in Tehran

Catheter-associated urinary tract infection (CAUTI) remains an important healthcare-associated infection in hospitalized patients, particularly in intensive care and other high-risk wards. This descriptive study aimed to characterize the clinical and microbiological features of CAUTI among ۵۰ hospitalized patients in a tertiary hospital in Tehran. Patient data were obtained from medical records and the hospital information system. The analyzed variables included age, sex, ward category, interval from admission to infection onset, interval from urinary catheter insertion to infection onset, length of hospital stay, urine culture result, available antimicrobial susceptibility findings, and in-hospital outcome. The study included ۲۳ male and ۲۷ female patients, with a mean age of ۶۳.۴۶ ± ۱۵.۷۹ years. Most cases were identified in intensive care units. Candida spp. was the most frequent organism, accounting for ۲۶ cases (۵۲.۰%), followed by Escherichia coli in ۱۱ cases (۲۲.۰%), Klebsiella spp. in ۹ cases (۱۸.۰%), and Enterococcus spp. in ۴ cases (۸.۰%). Among bacterial isolates, resistance was observed across several antimicrobial classes, especially among Klebsiella spp. and Escherichia coli isolates. In-hospital mortality was recorded in ۲۵ patients (۵۰.۰%); however, death could not be directly attributed to CAUTI because underlying diseases, disease severity, comorbid conditions, or concurrent infections may have contributed to patient outcomes. Overall, the findings highlight the importance of local CAUTI surveillance, careful urinary catheter management, and regular review of antimicrobial resistance patterns in hospital settings
