A comparison of Perioperative Blood Loss between Simple Suprapubic Prostatectomy and Transurethral Resection of Prostate, based on Pre- and Post-Operative Hemoglobin Level Differences

Introduction: Benign prostatic hyperplasia (BPH) is one of the most common urological conditions among elderly men, often requiring surgical intervention in advanced stages. The two main surgical techniques, simple suprapubic prostatectomy and transurethral resection of the prostate (TURP) that each have their own advantages and disadvantages. This study aimed to compare the amount of intraoperative and postoperative bleeding between simple suprapubic prostatectomy and TURP in patients admitted to Ayatollah Mousavi Hospital in Zanjan from ۲۰۲۰ to ۲۰۲۲.Methods: This descriptive-analytical cross-sectional study was conducted on ۱۳۲ hospitalized patients. Data regarding demographic characteristics, type of surgery, prostate volume, duration of surgery, and blood indices (hemoglobin, hematocrit, and platelet count) before and after surgery were extracted from patient records and analyzed using independent t-test.Results: Of the ۱۳۲ patients, ۸۴.۵% underwent TURP and ۱۵.۵% had simple suprapubic prostatectomy. The reduction in hemoglobin and hematocrit levels was significantly greater in the simple suprapubic prostatectomy group compared to the TURP group (P-value<۰.۰۰۱). There was no significant difference in platelet reduction between the two groups. Prostate volume and operative time were significantly higher in the simple suprapubic prostatectomy group.Conclusion: The results indicated that TURP is associated with less bleeding compared to simple suprapubic prostatectomy and may be the preferred method for patients with moderate prostate volume. Surgical method selection should be based on the patient's condition, prostate size, and available healthcare resources. Further multi-center studies and evaluation of hemostatic medications are also suggested.
