Vulvar Paget Disease: A Case Report and Literature Review

Vulvar Paget's Disease (VPD) is a rare skin disease mainly found in post-menopausal women. The standard treatment for VPD involves surgically removing the lesions with wide margins. However, this approach often leads to cosmetic dissatisfaction and possible complications that do not guarantee the prevention of local recurrence. Here, we report a ۷۵-year-old multiparous woman who presented with a pruritic lesion on the vulva that had been ongoing for two years. A punch biopsy confirmed the diagnosis of Paget's disease of the vulva. The initial treatment involved a radical vulvectomy with a one-centimeter margin, preserving the clitoris. During the procedure, frozen section control was used to assess the margins. Subsequently, re-excision and bilateral inguinofemoral lymphadenectomy were performed. To address the vulvar defect, rectus abdominis myocutaneous flaps were utilized. During the surgery, rectus abdominis myocutaneous flaps were used. The patient had a ۶-month follow-up period that no recurrence was observed.
