Correlation between ۲۵-hydroxyvitamin D and severity of knee osteoarthritis in patients referred to Rafsanjan rheumatology clinic in ۲۰۲۲

Osteoarthritis is one of the most important reasons of disability. This study was conducted to investigate the correlation between serum level of ۲۵-hydroxyvitamin D and the severity of disease in patients with knee osteoarthritis. In this descriptive (correlational) study, ۵۰۶ patients with knee osteoarthritis referring to the rheumatology clinic of Rafsanjan city during ۲۰۲۲ were investigated. Osteoarthritis was diagnosed by a rheumatologist according to the American College of Rheumatology criteria. Serum levels of ۲۵-hydroxyvitamin D were measured using a ۵ cc non-fasting blood sample drawn from the patients. Pain intensity was assessed using the Visual Analogue Scale (VAS), and functional status was evaluated using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Statistical analysis was performed with SPSS ۲۰ software with an alpha of ۰.۰۵. The patients' mean age was ۵۳.۷۵ ± ۹.۸۲ years and the frequency of women was ۷۸.۷% (۳۹۸ people). Mean VAS and WOMAC scale were significantly different at various vitamin D serum levels (P < ۰.۰۰۱). The results of the correlation analysis showed that VAS and WOMAC scores decreased significantly with increasing vitamin D levels (P < ۰.۰۰۱). Based on the results of multiple linear regression model, WOMAC scale was ۱.۳۳۸ points lower in osteoarthritis patients with vitamin D deficiency (P = ۰.۰۲۴). The results of the present study demonstrated that serum ۲۵-hydroxyvitamin D levels were associated with disease severity in patients with osteoarthritis. These findings suggest that optimizing vitamin D levels may have beneficial and clinically significant effects on improving pain and functional status.
