Comparison of the Accuracy of Three Instruments (APACHEII, SOFA, and MPMII) in Predicting the Mortality of Patients in Intensive Care Units

Accurate prediction of mortality in intensive care units (ICUs) is vital for patient management and resource allocation. Among the most widely used scoring systems are APACHE II, SOFA, and MPM II, each providing valuable but variable estimates of patient outcomes. The present study aimed to compare the accuracy of these three instruments in predicting mortality in ICU patients. This cross-sectional study (۲۰۲۰) included ۱۵۰ ICU patients from Ardabil University hospitals. Eligible participants were adults (>۱۸ years) with ICU stays ≥۲۴ hours, excluding burn and cardiac surgery cases. Data were gathered using demographic forms and APACHE II, SOFA, and MPM II questionnaires. Statistical analyses (SPSS v۲۳) applied descriptive statistics, t-test, correlation, sensitivity, and specificity. Among the studied patients, ۸۵ were male, with a mean age of ۶۰.۷۴ ± ۱۹.۶۲ years. The average duration of hospitalization was ۱۰.۱۷ ± ۱۰.۳۴ days, and ۵۲ patients died. Logistic regression showed that hospital type, gender, age, length of hospitalization, duration of intubation, diagnosis, and GCS were not significant predictors of mortality. Predicted mortality was ۳۲.۲۱% with APACHE II, ۳۲.۵۰% with SOFA, and ۶۴.۰۱% with MPM II. Sensitivity and specificity values were ۸۳% and ۴۸% for APACHE II, ۷۹% and ۴۶% for SOFA, and ۷۳% and ۵۰% for MPM II, respectively. In conclusion, APACHE II demonstrated higher accuracy compared with SOFA and MPM II, and its application is recommended as the preferred prognostic tool in ICU settings.
