Psychometric properties of Persian version of PTSD questionnaire for phone survey: An experience in PERSIAN Traffic Cohort

Objectives: Almost ۵۰ million people experience road traffic accidents (RTA) annually. Such traumatic events are mostly followed by long-lasting, weakening Posttraumatic Stress Disorder (PTSD). We aimed to investigate the psychometric properties of PTSD-۵ checklist at phone follow-ups and determine its applicability in screening in RTA survivors. Methods: We recruited ۱۵۰ patients and assessed reliability, applicability and agreement within a national cohort platform, at one-, six-, and twelve-month follow-ups after discharge, enrolling ۵۰ patients per phase. The average duration between test and retest was ۱۰ days. We measured internal consistency reliability and stability reliability, as well as agreement using Kappa coefficients, percentages of agreement and Bland-Altman method. Data were analyzed using STATA statistical software package version ۱۵. Results: The participants were mostly male (۷۹.۸%; mean age: ۴۱ (SD:۱۴.۷)). Above ۷۰% of participants were married or employed or educated. The Persian version had excellent internal consistency reliability at total and follow-up levels (Cronbach's _x۰۰۰۳_ = ۰.۹۵) as well as good total stability reliability (ICC = ۰.۷۷, ۹۵% CI: ۰.۶۸, ۰.۸۳). One month follow-up data showed poor stability reliability (ICC =۰.۳۲, ۹۵% CI: -۰.۱۰, ۰.۵۹). The test-retest agreement at six- and ۱۲-months was excellent. Total Kappa coefficient (۰.۵۸) and percentage (۷۱.۳%) revealed moderate agreement. The one-year Bland Altman plot revealed more agreement at healthier state (lower scores). Conclusion: The Persian PTSD checklist was highly reliable and applicable through phone interviews post discharge in road traffic injury patients at ۱۸ years old or above. It was suitable to apply within the national cohort study and to decline the data collection costs.
