Abstract
Background: Occupational Health Examinations (OHEs) are fundamental
components of workplace health surveillance systems. In Iran, the Ministry of
Health and Medical Education (MoHME) has defined regulatory standards for
periodic examinations, yet limited evidence exists regarding the quality of
implementation and workers’ satisfaction. This case study evaluates compliance
of OHEs with MoHME standards, worker satisfaction, and the preventive impact
on musculoskeletal disorders (MSDs) and occupational diseases among
industrial and trade workers.
Methods: A cross-sectional case study was conducted among 500 workers from
metal, construction, textile, food, and small trade sectors. Data were collected
through structured questionnaires, interviews, and review of OHE records. Key
quality indicators included compliance with MoHME OHE standards, coverage
and frequency of examinations, comprehensiveness of clinical evaluations,
ergonomic assessments, follow-up procedures, and communication of results.
Descriptive and inferential analyses (ANOVA and chi-square) were performed.
Results: Overall compliance with MoHME standards was 72.4%. The highest
compliance was observed in metal industries (84.1%), and the lowest in small
trades (61.7%). Worker satisfaction was moderate (65.8%), with the highest
satisfaction in examination professionalism and the lowest in ergonomic
feedback and preventive recommendations. Musculoskeletal complaints were
significantly lower among workers receiving comprehensive ergonomic
evaluations . Preventive follow-up improved early detection of
hypertension, hearing impairment, and lower back disorders.
Conclusion: While OHE quality in Iran shows acceptable alignment with national
standards, major gaps remain in ergonomic assessment, worker education, and
follow-up. Strengthening training programs for occupational physicians,
reinforcing regulatory supervision, and enhancing feedback mechanisms can
improve preventive outcomes.
