Factors Affecting Medication Adherence in Patients with Heart Failure: A Systematic Review

Objective: This research aims to identify the key factors influencing medication adherence among individuals diagnosed with heart failure. we searched PUBMED, Web of Science, and Scopus from January ۲۰۱۴ to October ۲۰۲۴ to locate the most recent original cross-sectional studies that addressed the factors affecting medication adherence in patients with heart failure. Results: Of ۱۲۳ identified records, ۱۵ studies met inclusion criteria. Factors influencing adherence were grouped into four domains: demographic (education level, marital status, socioeconomic status), treatment- and disease-related (polypharmacy, comorbidities), psychological (depression, anxiety, self-efficacy), and patient-related (health literacy, forgetfulness, patient-provider relationship). Higher education, social support, and good communication with providers were associated with improved adherence, while financial constraints, pill burden, and psychological distress reduced adherence. Conclusion: Medication adherence is multifactorial and modifiable. Understanding these factors can guide nurses and clinicians in designing targeted interventions to improve adherence and outcomes in heart failure patients.
