The Relationship Between Cold and Hot Mentalizing and Self-Punishment Patterns in Individuals with Antisocial Personality Traits: A Meta-Analysis .

Antisocial Personality Disorder (ASPD) is characterized by a persistent disregard for the rights of others, yet the internal psychological mechanisms, particularly the interplay between mentalizing and self-directed aggression, remain complex. This meta-analysis investigates the relationship between “Cold” (cognitive) and “Hot” (affective) mentalizing and self-punishment patterns in individuals with antisocial traits. A systematic search of literature from ۲۰۲۰ to ۲۰۲۵ (including English and Persian sources) was conducted via Google Scholar. Ten studies meeting the inclusion criteria were analyzed. Results indicate a significant negative correlation between Cold mentalizing and antisocial traits (r = -۰.۳۵, p < ۰.۰۰۱), while Hot mentalizing showed a weak or inconsistent relationship. Furthermore, a strong positive correlation was found between mentalizing deficits and self-punishment patterns (r = ۰.۴۴, p < ۰.۰۰۱). These findings suggest that while antisocial individuals may cognitively understand others’ states (Cold mentalizing), their inability to integrate affective components (Hot mentalizing) leads to maladaptive self-regulatory behaviors, including self-punishment. Clinical implications for Mentalization-Based Treatment (MBT) are discussed.
