Background: Stringhalt is a neuromuscular disorder in horses that causes abrupt hyperflexion of the hind limb, particularly when backing or turning. It can be acquired, often associated with toxic pasture, or idiopathic. Diagnosis is approved based on clinical signs. The outcomes vary based on the treatment, which could be pasture removal or surgical intervention. Methods: A ۶-year-old Turkmen stallion was presented in winter ۲۰۲۴. It had been a month since abnormal hindlimb gait was observed which was characterized by exaggerated flexion. The owner reported that no improvement was shown by previous treatments, including calcium supplementation. Clinical examinations confirmed stringhalt, defined by abrupt hyperflexion during the swing phase, particularly when turning or backing. The condition can range from mild intermittent lifting to severe jerking movements. The chronic cases can lead to stumbling or muscle atrophy. Non-painful palpation and normal systemic parameters can aid in the differential diagnosis among shivering, upward fixation of the patella, and peroneus tertius rupture. Given the lack of response to medical management, a lateral digital extensor myotenectomy was performed. This particular surgery is the preferred treatment for idiopathic or refractory cases and can significantly reduce abnormal flexion, although complete recovery is not always guaranteed. Results: The surgery led to remarkable outcomes. Following surgery, the stallion recovered completely with no recurrence of abnormal hind limb flexion. Conclusion: This case demonstrates the clinical effectiveness of lateral digital extensor myotenectomy in the management of stringhalt. In this horse, timely recognition of the disorder and surgical intervention led to complete recovery. The outcome emphasizes how early diagnosis and appropriate treatment can restore mobility and quality of life in horses suffering from this gait abnormality.
