Uterine Leiomyoma Associated with Closed-Cervix Pyometra in a German Shepherd Bitch: A Case Report

Background: Uterine smooth muscle tumors (leiomyomas) are uncommon, benign neoplasms that occur mainly in intact female dogs. Clinical manifestations may resemble those of uterine inflammatory diseases such as pyometra. The present report describes a large uterine leiomyoma diagnosed during the evaluation and surgical treatment of a suspected pyometra case. Case presentation: A ۹ year old intact female German Shepherd dog weighing ۳۹ kg was referred with lethargy, anorexia, abdominal distention, and mammary secretions. Based on initial findings, closed-cervix pyometra was suspected. Ultrasonography revealed cystic endometrial hyperplasia (CEH) with pyometra and a large cavitary intra abdominal mass (approximately ۱۶ cm in diameter). Thoracic radiographs exhibited no signs of metastasis or cardiomegaly. An ovariohysterectomy was performed, and the excised uterus contained a massive cavitated mass weighing ۴.۸ kg with uterine horns measuring ۵ cm in diameter. Histopathological examination demonstrated well differentiated smooth muscle cells consistent with benign uterine leiomyoma measuring ۲۳ × ۲۴ cm. Result & Conclusion: The postoperative course was uneventful. The patient recovered and was discharged with favorable prognosis. No clinical complication was observed during follow up. This case demonstrates that uterine leiomyoma in intact female dogs may clinically mimic pyometra. Comprehensive imaging and histopathological analysis are essential for accurate diagnosis. Surgical management via complete ovariohysterectomy remains the treatment of choice and provides an excellent prognosis.
