Computed tomographic characterization of a single extrahepatic splenocaval shunt with nephrolithiasis in a Shih Tzu: a CARE-compliant case report

Background: Extrahepatic portosystemic shunts (EHPSS) are vascular malformations that divert portal blood from the liver, causing metabolic and urinary complications. Computed tomographic angiography (CTA) is now the gold standard for anatomic diagnosis and surgical planning in affected dogs. Methods: A CARE-compliant case report was compiled from clinical records, laboratory data, and diagnostic imaging of a ۵-year-old spayed female Shih Tzu presented with lethargy and hematuria. Results: Hematology was within reference limits but showed microcytosis and hypochromia. Serum biochemistry indicated hypoalbuminemia, hypocholesterolemia, and mildly elevated ALT. Radiography and ultrasonography demonstrated marked microhepatia and mineralized foci compatible with renal and bladder calculi. CTA revealed a single tortuous splenocaval shunt connecting the splenic vein and caudal vena cava. Two uric acid bladder stones (۰.۱ g) were later surgically removed. Conclusion: CTA enabled definitive diagnosis of splenocaval EHPSS with urolithiasis, emphasizing its diagnostic precision and the importance of urinary surveillance before surgical correction.
