Background: Chronic low-grade inflammation is a frequent comorbidity in chronic kidney disease (CKD) and is especially common among individuals receiving long-term dialysis. Interleukin-۶ (IL ۶) is a key inflammatory mediator that drives systemic inflammation, disrupts immune homeostasis, and contributes to complications such as cardiovascular disease and the accelerated progression of renal failure. Methods: In this pilot investigation, we examined serum IL ۶ patterns in five adult cats with CKD treated with colonic dialysis as an alternative form of renal replacement therapy. Over ۳۵ days, each cat received a modified peritoneal dialysis solution enriched with mannitol and activated charcoal, administered via the colon. Blood samples were collected on days ۰, ۱, ۳, ۱۴, ۲۱, and ۳۵ to quantify IL ۶ levels. Results: Across the study period, mean serum IL ۶ concentrations rose from baseline to day ۳۵, indicating a gradual yet persistent inflammatory response during treatment. This sustained elevation suggests that colonic dialysis, while potentially alleviating certain renal symptoms, may be insufficient to control systemic inflammation in cats with CKD particularly in those with chronic underlying pathology. Conclusion: These findings highlight IL ۶ as a promising independent biomarker for monitoring inflammation in feline CKD, with potential clinical applications in guiding therapy, identifying high-risk patients, and shaping strategies to target inflammation alongside renal replacement treatments.
