Post-Hepatic Jaundice Secondary to Platynosomum Infection in a ۱.۵-year-old Domestic Cat: A Case Report

Background: Feline hepatitis and post-hepatic jaundice syndromes are multifactorial illnesses with multifarious infectious as well as obstructive etiopathogenesis. Platynosomum fastosum is one among parasitic etiologies being a grand trematode to produce pleioclitic bile duct obstruction and chronic cholangitis. Methods: ۱.۵-year-old, neutered, male domestic shorthair (DSH) cat that was ۸ kg and lived outside most of the time presented with jaundice, fever, and ataxia. A full diagnostic work-up was performed, i.e., hematology, serum biochemistry, abdominal radiography, and ultrasonography. Laboratory examinations revealed severe hepatocellular damage and cholestasis. After excluding other differential diagnoses such as hepatic lipidosis, neoplasia, and extrahepatic bile duct obstruction, Platynosomum fastosum infection was assumed on imaging and clinical grounds. Supportive hepatic treatment was initially attempted but did not show any improvement in the clinical signs. The patient was then administered praziquantel at a dose rate of ۴۰ mg/kg orally once daily for five consecutive days. Results: After praziquantel treatment, the cat showed rapid improvement clinically. Within a week, neurological signs and jaundice resolved entirely. In follow-up laboratory exams, normalization of activities of hepatic enzymes and bilirubin levels was observed, reflecting restoration of liver function. Radiographic and ultrasonographic reassessment revealed resolution of hepatomegaly and biliary compression. The patient regained normal appetite, activity, and body weight and was clinically stable during follow-up assessments. Conclusion: Here, the necessity of considering trematode infection in differential diagnosis of feline hepatitis and post-hepatic jaundice is illustrated. Prompt anthelmintic treatment will correct liver injury and avoid biliary fibrosis.
