Background: Cryptorchidism is a common reproductive disorder in stallions, and the condition can present concurrently with other abdominal pathologies such as colic. This report describes the diagnosis and surgical management of a horse presenting with bilateral cryptorchidism and colic. Methods: A ۶-year-old Kurdish stallion with signs of severe colic underwent exploratory celiotomy under general anesthesia. Concurrently, bilateral cryptorchidectomy was performed via midline and inguinal approaches Results: The horse received surgical treatment for colic as well as bilateral cryptorchidism that was successful. While the horse was under anesthesia, the horse's abdomen was entered and findings included distension of the cecum and displacement of the colon, which were corrected without complications. The contents in the ventral and dorsal colon were passed through a pelvic flexure incision, and both retained testes, one in the abdomen and one in the inguinal region, were located, ligated, and excised without intraoperative hemorrhage. The midline abdominal incision and inguinal incisions were closed routinely in three layers. The horse woke smoothly from anesthesia, and there were no immediate complications postoperatively. The surgery was effective in resolving the colic episode and cryptorchidism in the same surgical procedure. Conclusion: This situation shows the effective simultaneous surgical treatment of severe colic and bilateral cryptorchidism as one surgical procedure. In this case, prompt recognition of colic and treated retained testicles were ultimately successful, leading to a positive outcome with minimal risk for long-term complications. This case highlights the need for comprehensive preoperative assessment and coordinated surgical intervention in horses with multiple combined abdominal and reproductive diagnoses.
