Objective and Background: In general, atherosclerotic heart diseases remain a major health problem worldwide despite efforts to manage and treat them. Recently, epigenetic studies have revealed that certain types of microRNA can serve as novel biomarkers in the diagnosis and prevention of this condition. miRNAs are small, non-coding RNA molecules with an approximate length of ۱۶-۲۲ nucleotides that regulate gene expression at the post-transcriptional level by targeting mRNA molecules and among these, miR-۱۲۶ can be mentioned, which is an intronic molecule located in the EGFL۷ gene on human chromosome ۹. The present epidemiological study was conducted with the aim of determining the role of miR-۱۲۶ in the diagnosis and prevention of atherosclerotic heart disease. Materials and Methods: This study is a review and the result of searching internet databases such as Pubmed, Google scholar, and Scopus between ۲۰۱۵ and ۲۰۲۴. Articles that had any of the keywords miR-۱۲۶, microRNA, Atherosclerosis, Coronary Artery disease (CAD) and the full text of the articles were available; were included in the study. Initially, more than ۱۵ articles were included in the study, and finally, by removing duplicates in the Endnote program, twelve articles were included in the study and used for analysis. Results: Studies showed that the expression level of miR-۱۲۶ was reduced in patients with CAD and some other heart failure compared to healthy individuals, and its higher level indicated a better clinical condition of the patient. It was also found that the expression level of this molecule was negatively correlated with the severity of the disease and the level of natriuretic peptide type B (NPB). On the other hand, it was inferred that increased expression of miR-۱۲۶ leads to inhibition the level of vascular cell adhesion molecule-۱ (VCAM-۱) during inflammation and protection against the occurrence of atherosclerosis. Conclusion: Based on the studies conducted, the prognostic and diagnostic capabilities of miR-۱۲۶ for heart and coronary artery diseases can be well deduced, and even its relationship with common risk factors for cardiovascular diseases such as blood pressure and even diabetes can be observed, but it also requires further investigation. Also, increased expression of this microRNA could emerge as a novel therapeutic target in atherosclerotic cardiovascular diseases due to its potential in promoting angiogenesis and anti-inflammatory effects.
