Prenatal stress and anxiety are significant contributors to adverse birth outcomes, including preterm birth and low birth weight. This study aimed to systematically evaluate the efficacy of Mindfulness-Based Interventions (MBI) in mitigating pregnancy-related anxiety and subsequently improving obstetric results. Methods: A systematic review was conducted, exclusively including randomized controlled trials (RCTs) published since ۲۰۱۵. Primary outcome measures included standardized anxiety scores (e.g., STAI) and delivery indicators (e.g., gestational age at delivery and birth weight). Data were systematically extracted from Medline and PsycINFO databases following PRISMA guidelines. Results: Sixteen RCTs met the inclusion criteria. MBI programs demonstrated a significant reduction in third-trimester anxiety scores (mean reduction of ۷.۳ points on a ۵۰-point scale) (p<۰.۰۱). This observed stress reduction correlated with a statistically significant decrease in the risk of preterm labor (by ۰.۵ units) and a mean increase of ۹۰ grams in birth weight compared to control groups. Discussion & Conclusion: Strong evidence supports MBI as a safe and effective strategy for enhancing maternal mental well-being, which translates into positive physical birth outcomes. Integrating these low-cost, non-pharmacological approaches into routine antenatal care protocols warrants strong recommendation for clinical implementation.
