چکیده مقاله:
Chronic conditions such as cardiovascular diseases, diabetes, chronic kidney disease, and chronic obstructive pulmonary disease (COPD) contribute to over ۷۰% of deaths worldwide. Timely diagnosis is crucial for effective treatment, but traditional approaches are reactive and lead to late diagnosis. IoT and AI technologies offer continuous monitoring, timely anomaly detection, and individualized interventions. This review examines ۳۰ high-quality articles from ۲۰۲۳-۲۰۲۵ on IoT sensors, wearables, edge-cloud systems, and AI algorithms like machine learning and deep learning. Opportunities include real-time monitoring, individualized diagnosis, remote patient observation, accurate diagnoses, and reduced healthcare costs. Challenges include privacy issues, data security, interoperability, energy limitations in IoT devices, ethical concerns, and opacity of AI decision-making. Solutions like federated learning and blockchain-enabled secure sharing are proposed. Interdisciplinary partnerships are essential for responsible integration of IoT and AI technologies in chronic disease care.
