Airports serve as primary operational environments for aircraft and are vital components of the aviation industry. Studies of aviation accidents and incidents worldwide indicate that over ۷۵% of these events occur within airport environments, including operational surfaces and surrounding traffic periods. This descriptive-analytical study conducts a Bowtie analysis of vehicle collisions with stationary aircraft on the apron of one of Iran's international airports. Data were collected through direct observations and interviews with experts during brainstorming sessions. The collected data provided a comprehensive analysis of the causes, barriers, consequences, and potential mitigations related to airport operations, emphasizing safety and operational quality, especially in apron management and driver performance. Various challenges arose from inadequate regulations, inefficiencies in systems, cultural resistance, and poor training, all of which affected employee performance and operational integrity. To improve safety and quality, a multifaceted approach had been necessary, encompassing strengthened regulations, enhanced training, fostering a positive safety culture, and investing in infrastructure to reduce risks and improve overall operational efficiency. Damage to the aircraft fuselages, damage to the vehicle and lost reputation of the airport or handler are acceptable based on the safety risk mitigation and it may require management decision. And resulted in death or injury is acceptable.
