Wound care especially for chronic or hard-to-heal lesions remains a major clinical challenge. In this context, drug nanocarriers based on hierarchical zeolites act as the “smart core” of advanced dressings: their multi-scale micro-/mesoporous network offers exceptionally high loading capacity for antibiotics, antimicrobial ions, and growth factors, while regulating the targeted, sustained release of these agents. When these crystalline nanocarriers are dispersed within a biocompatible hydrogel matrix, the hydrogel maintains moisture and intimate contact with tissue, providing the ideal platform for the nanocarriers’ staged performance simultaneously suppressing infection, controlling inflammation, and accelerating tissue regeneration. This review elucidates the key features of zeolite-based drug nanocarriers and their synergy with hydrogels, compiles the latest fabrication methods and in vitro/in vivo data, discusses translational hurdles and regulatory pathways, and outlines future prospects for stimuli-responsive and multifunctional systems. Finally, emerging directions including multi-triggered release systems, gene-editing payloads, and integration with real-time biosensors are mapped to illustrate how zeolite–hydrogel nanocarrier platforms could reshape next-generation precision wound management.
