چکیده مقاله:

Background: Cardiovascular diseases are a leading cause of morbidity and mortality worldwide, necessitating innovative solutions for patient management. Remote monitoring (RM) has emerged as a promising strategy for continuous, patient-centered cardiac care. Methods: In this cross-sectional interventional study conducted in Urmia, Iran, cardiac patients discharged from hospital were enrolled and provided with wearable monitoring devices and a secure mobile application. Clinical data were transmitted daily to a centralized nursing team for review. Primary outcomes included the rate of unplanned hospitalizations, emergency department visits, and all-cause mortality, with secondary outcomes assessing patient satisfaction, usability, and quality of life. Results: Over ۹۰ days, the RM group exhibited lower rates of unplanned hospitalization (۱۰% vs. ۲۳.۳%) and ED visits (۱۳.۳% vs. ۳۳.۳%) compared to standard care. Patient satisfaction, device usability, and self-management confidence were significantly higher in the RM group. The intervention was well-received, demonstrating feasibility and effectiveness in an Iranian context. Conclusion: Remote monitoring enhances nursing care for cardiac patients by improving outcomes and patient engagement. Adoption of RM strategies can contribute to safer, more efficient, and more personalized cardiac care.
