Effectiveness of Nurse-led Interventions in the Management of Diabetes, Hypertension and COPD: A Systematic Review

Given the growing burden of chronic diseases such as diabetes, hypertension, and chronic obstructive pulmonary disease (COPD), there is an increasing need for effective and cost-efficient health interventions. This systematic review aimed to evaluate the effectiveness of nurse-led interventions in the management of these prevalent chronic conditions. A comprehensive literature search was conducted in PubMed, Scopus, and Cochrane databases from ۲۰۱۰ to ۲۰۲۵, resulting in the inclusion of ۲۴ studies comprising randomized controlled trials (RCTs) and cohort studies. The findings revealed that nurse-led interventions in diabetic patients significantly reduced HbA۱c levels and improved self-management behaviors. In patients with hypertension, improvements in systolic and diastolic blood pressure control were observed. Among COPD patients, the interventions led to fewer hospital readmissions, enhanced quality of life, and better adherence to treatment. Despite heterogeneity in study designs and intervention strategies, the majority of studies reported favorable outcomes. These results underscore the pivotal role of nurses in improving clinical and economic outcomes across chronic disease management.
