Introduction The climate crisis severely impacts human health, particularly pregnant women, mothers, and children. Midwives, as primary MCH providers, are uniquely placed to address these impacts, especially in resource-limited areas. This review clarifies midwife-led climate resilience, showing how their expertise and community ties empower MCH adaptation in highly vulnerable regions. Methodology This review synthesized evidence from major scientific databases (PubMed, Scopus, Global Health, Web of Science) and from years ۲۰۲۰-۲۰۲۵. reports from key organizations (WHO, UNFPA, UNDP, ICM). Keywords focused on 'climate change,' 'maternal health,' 'child health,' and 'adaptation.' Findings Midwives are crucial for MCH climate resilience. They provide frontline care with minimal resources, act as community educators on health risks, and are key in emergency response. They also advocate for MCH in climate plans and promote sustainable healthcare. Discussion and Conclusion Current evidence shows midwives are essential, though often unrecognized, for MCH climate adaptation. They bridge global climate issues with local health needs, especially for vulnerable groups. Investing in their training, support, and integration into climate plans is vital for strengthening health systems and protecting maternal and child health against
