Effect of a Modified Technique of Transurethral Resection of Prostate on Ejaculation and Continence

Introduction: To study the effect of preservation of supramental tissue in preservation of continence and ejaculatory function in patients with benign prostatic hyperplasia (BPH) undergoing trans-urethral resection of prostate (TURP).Methods: This prospective comparative study was conducted in patients with benign prostatic hyperplasia having prostate size between ۳۰ and ۶۵ cc with following indications for surgery: lower urinary tract symptoms refractory to medical therapy, gross hematuria due to BPH, multiple failed voiding trials and recurrent urinary tract infections. All patients were sexually active with preserved ejaculation before surgery. A total of ۱۶۴ patients were included in the study and randomly divided into either the modified TURP group or the conventional TURP group. In the modified TURP group the distal extent of resection extended only approximately ۱ cm proximal to the verumontanum. Preoperative data like age, volume of prostate, serum prostate specific antigen, maximum urinary flow rate, international prostate symptom score and international index of erectile function score were comparable between the two groups.Results: The difference in transient incontinence and ejaculation preservation rate between the two groups was statistically significant. The mean operative time, resected prostatic tissue, volume of irrigation fluid used, and the hemoglobin drop were all less in the modified TURP group as compared with the conventional TURP group, but none of the differences were statistically significant.Conclusion: The modification of TURP proposed in our study seems to offer a better alternative in terms of preservation of continence and ejaculatory function in patients undergoing TURP, while maintaining a satisfactory voiding function.
