Comparison of Norepinephrine ۱۶ µg and Ephedrine ۱۰ mg for Attenuating Post-Intubation Hypotension in Intensive Care Unit Settings

Post-intubation hypotension is common in patients in the intensive care unit (ICUs). In some cases, it can even lead to death shortly after surgery. This study aimed to compare the effectiveness of two vasopressors, norepinephrine (NE) and ephedrine (ED), in preventing post-intubation hypotension in the intensive care unit (ICU). This multicenter, double-blind, randomized clinical trial will be conducted between January and June ۲۰۲۴. The inclusion criteria were as follows: patients aged ۱۸-۶۴ years who were scheduled for intubation and agreed to participate in the study. The exclusion criteria included patients in the ICUs who received vasopressor treatment before the beginning of the study. The patients were divided into two groups: the NE group received a bolus of ۱۶ µg NE intravenously, and the ED group received a bolus of ۱۰ mg ED. Blood pressure was measured before intubation, during intubation, and at the ۱st, ۳rd, ۶th, and ۱۰th minutes after intubation. A total of ۴۴ participants were included in the final analysis (۲۲ participants per group). Although no statistically significant difference was observed in the systolic blood pressure (SBP) between the NE and ED groups, significant differences were found in the diastolic blood pressure (DBP) and mean arterial pressure (MAP). NE was more effective than ED in controlling the DBP and MAP. Additionally, NE use resulted in a slight increase in blood pressure and less tachycardia than that observed in the ED group. Both NE and ED can be used as vasopressors to prevent post-intubation hypotension in ICU patients, with NE showing superior control over DBP and MAP compared with ED. These findings suggest that NE may be more effective than ED in maintaining hemodynamic stability after intubation
