Background and Aims: The global healthcare system has evolved from diagnosing and treating patients to providing personalized care. This shift highlights technology's role as a competitive factor. Healthcare ۴.۰ has emerged from digitalization, transforming the industry with artificial intelligence, data analysis, and real-time medical care. Artificial Intelligence can extract valuable information from medical records and clinical reports. This technology is vital in Healthcare ۴.۰, but its application requires new infrastructure and guidelines within the healthcare sector. Preparing for the widespread adoption of this technology necessitates careful planning, implementation, and continuous monitoring. Developing reliable training courses and guidelines for utilizing artificial intelligence in medical sciences demands collaboration among all stakeholders. Given the expansion of AI applications in healthcare and the challenges and potential barriers, policymaking and regulation of this emerging technology have received heightened attention in recent years. Despite significant advancements in artificial intelligence, the healthcare industry in Iran still holds many opportunities for digital solutions. Methods: The initial phase involves a systematic review to examine the current AI regulations in healthcare across ten prominent organizations and countries. Articles from ۲۰۰۷ to ۲۰۲۵ were reviewed for research variable relationships, along with current guidelines. Due to the topic's novelty, ۲۳ relevant articles were found. No prior research explored these connections in Healthcare ۴.۰. The guiding principles of Iran's National Document on Artificial Intelligence were also examined. The study's second phase will analyze artificial intelligence policy in healthcare using the health policy triangle framework, which emphasizes content, process, context, and actors. Results: Based on policy analysis and stakeholder insights, this research will recommend guiding principles for AI policy actions in the Iranian healthcare industry. It will also suggest challenges and strategies for integrating AI in health management across planning, implementation, and continuity in the Strategic Plan Implementation model. Conclusion: Generative AI faces misconceptions and misuse. Clear healthcare regulations are crucial. An organization's readiness affects its technical and regulatory preparedness in AI integration. Establishing clinical criteria and selecting appropriate methodologies are key for optimal outcomes. Trust is essential to avoid inequities. Future articles may further explore these issues.
