Borderline Personality Disorder (BPD) is a complex psychiatric condition marked by emotional dysregulation, interpersonal difficulties, impulsivity, and empathy deficits. These characteristics often lead to significant impairments in personal and social functioning. The present study examines the effects of interpersonal interactions within group therapy on improving empathy, reducing psychological conflicts, and enhancing behavioural control in individuals diagnosed with BPD. A quasi-experimental design was employed, using pre-test and post-test measures to assess therapeutic outcomes. Fifty participants, aged ۱۸–۴۵, were recruited from clinical centers in Tehran. The intervention included a ۱۲-week group therapy program emphasizing empathy exercises, conflict resolution skills, and behavioural regulation techniques. Findings revealed significant improvements in the experimental group, including enhanced empathy (۲۲%), decreased psychological conflicts (۱۵% reduction), and improved impulse control (۲۸%). Statistical analyses confirmed notable differences between the experimental and control groups, with p-values indicating strong significance (<۰.۰۱). These results highlight the social learning and emotional connection fostered within group therapy sessions as a critical factor driving behavioural and psychological improvements. Additionally, the gradual integration of interpersonal feedback helped participants develop greater self-awareness and trust in others. The therapy also facilitated long-term habits of behavioral regulation through shared accountability among group members. Participants expressed an increase in emotional intelligence and adaptability in relational settings, which contributed to improved therapeutic outcomes. Furthermore, the structured nature of group therapy provided a safe environment for practicing self-regulation and empathy strategies, ensuring consistent progress. The findings underline the importance of addressing core interpersonal challenges in BPD as part of a holistic therapeutic approach. The study urges clinicians and policymakers to consider group therapy as a cornerstone of treatment for BPD, given its capacity to address fundamental relational and emotional deficits. Future research should examine its applicability across diverse populations and its influence on long-term recovery outcomes.
