Background and purpose: Episiotomy as a method to widen the pelvic outlet is associated with complications such as pain and injury to the perineum, so the present study aims to evaluate the effect of honey on Pain intensity and healing of episiotomy wound. Materials and methods: In this systematic review study, the databases of Scopus, PubMed, SID, Medlib, Magiran, Web of Science and Google scholar were searched using the keywords of 'pain', 'Wound', 'Healing', 'Episiotomy', 'Perineum' and 'Hony' by OR and AND without time, language limitation until August ۲۰۲۴. The quality of the studies was evaluated using the JEDAD tool and data were analyzed by qualitatively. Results: In this study, ۲۰۸ articles were extracted. Then, seven clinical trials that met the study criteria were included in the study. The articles related to the effect of honey on the pain intensity and recovery of episiotomy wounds with a sample size of ۹۸۱ were reviewed. The results of these studies show that among the three reviewed studies on the effect of honey on episiotomy pain, only one study of honey reduces episiotomy pain. but regarding the effect of honey on wound healing, out of the five studies reviewed, in four studies, honey had a positive effect on episiotomy wound healing. Conclusion: honey improves episiotomy healing, and it seems that honey is more effective in healing episiotomy wounds than reducing its pain, although a definitive conclusion requires more studies.
