Introduction and Objective: Immobility can lead to pressure ulcers and skin breakdown, which can hinder mobility during treatment and recovery. Lack of movement while resting in bed can lead to bone loss, increased calcium excretion, and changes in the balance of electrolytes necessary for normal muscle function. Blood sugar levels increase when skeletal muscles are at rest, and insulin sensitivity decreases. One of the complications after surgery is pressure ulcers. The aim of this study is to present a rare case report of a Grade ۳ pressure ulcer in a patient after coronary artery bypass surgery, which completely healed in a short period of time despite the patient being mobile. Case Report: The patient was a ۶۰-year-old female with a history of diabetes. The patient had no mobility issues. During hospitalization after coronary artery bypass surgery, despite being mobile and getting out of bed for ۷ days in the hospital, she developed a pressure ulcer measuring ۶۶ cm with a depth of ۴ cm, Grade ۳, with an irregular margin in the sacral area. At the time of presentation, the patient's wound was necrotic and very dry, and due to the patient's high blood sugar levels, the treatment process was not good initially. She was then referred to an endocrinologist, and after the prescription of Lantus and NovoRapid insulin, it had a positive effect on the patient's recovery. The patient had no mobility issues during treatment and cooperated well with the therapist in the recovery process. The patient's wound completely healed after ۴۵ days and she had no complaints. Conclusion: The results of this case report indicate that one of the rare complications in patients with a history of diabetes after heart surgery, despite the patient having no mobility issues and being out of bed, is the development of a deep wound in the sacral area. The short-term recovery of this patient could be due to her mobility compared to other patients with larger wounds.
